Please fill out the form below to disconnect service with BCM One. For multi-site disconnect requests, please include a spreadsheet that includes all mandatory fields (Company name, Account number, Contact name, Email address, Phone, Address location, Service to be disconnected, Date to be disconnected, Reason for disconnecting) on the form for each location. Once the information is received, BCM One will confirm that the request is complete and that we do not require additional information to process your request. We will notify you once we determine that the form is complete, and the request is accepted. Your service(s) will be disconnected within 30 days from date of acceptance. Once the disconnect acceptance date is set, you will receive an email notification from one of our Order Management Systems (OMS) with the Disconnect Order number.
Please refer to your agreement with BCM One including the Standard Terms and Conditions for information related to your contract and any continuing obligations (including Early Termination Fees) arising from the disconnect of your service(s).
